Your Care Team

We care for every patient and family as we would our own. 

Medical Director

The patient’s plan of care is overseen by our medical director. Our Hospice Nurse Case Manager works closely with our medical director to coordinate care, communicate medication changes, treatments, and other medical needs outlined in the plan of care. Ongoing assessment findings are reviewed collaboratively to ensure timely adjustments to interventions, symptom management, and overall goals of care. Interdisciplinary team members are kept informed to promote continuity, patient safety, and care that remains aligned with the patient’s condition and expressed wishes.

Hospice Nurse

Our hospice nurses are highly experienced in assessing and managing pain, monitoring symptoms, and addressing end-of-life needs related to the patient’s terminal diagnosis. Working closely with the interdisciplinary hospice team and our Medical Director, the hospice nurse helps develop and coordinate the most appropriate plan of care. Regular visits are provided to ensure that the patient’s comfort, safety, and evolving needs are consistently met.

Social Worker

Our Hospice Social Worker provides compassionate counseling and emotional support to patients and families as they navigate the challenges associated with a serious illness. They assist with coping strategies, advance care planning, and guidance related to the patient’s diagnosis and changing needs. The Social Worker also helps connect families with community resources, including financial assistance, practical support, and funeral planning, ensuring that families feel supported both emotionally and practically throughout the hospice journey.

Home Health Aide/CNA

Provides in-home care to individuals who need assistance with daily living activities. They help with personal cares, such as bathing, dressing, and grooming, as well as with tasks like light housekeeping, and medication reminders. They also offer companionship and emotional support, helping patients maintain their quality of life wherever they call home.

Bereavement Coordinator

Our Bereavement Coordinator provides compassionate guidance and support to families before and after the loss of a loved one. Through individual outreach, grief education, and ongoing resources, they help families navigate the grieving process at their own pace. Whether offering a listening ear, connecting families to support services, or facilitating remembrance and healing, our Bereavement Coordinator is committed to ensuring no one has to walk through grief alone.

Chaplain

Hospice Chaplains provide non-denominational spiritual support while in our care. Chaplains can also work with the patient’s local clergy to create a broader scope of spiritual care for the patient and family.​

Other Services

Volunteers

Our Volunteers provide companionship. This may include but is not limited to: reading to patients, playing card games, or spending time conversing.

Therapy

At Elite Hospice we also understand the need for other services such as Physical Therapy, Occupational Therapy and Speech Therapy.

Life Enhancement Program

This program is designed to individualize patient care holistically based on patient's lifelong preferences and interests. This can include activities such as music enrichment, pet therapy and aromatherapy.

Little Things Program

Our Little Things program honors what matters most to each patient. Whether it’s a quiet fishing trip, a movie outing, or enjoying a favorite meal, these moments are about comfort, connection, and joy. Special experiences don’t have to be grand or one-time events—often, it’s the small, meaningful “little things,” woven throughout a patient’s hospice journey, that bring the greatest sense of peace and dignity.

Video Conferencing

Through secure software applications, we facilitate voice and video calls for patients. This allows patients and families to see one another and connect through meaningful, face-to-face conversations, even when they cannot be together in person.

Education & Support

We believe knowledge brings comfort and confidence during one of life’s most vulnerable times. Our team provides education and support for families and caregivers to help understand hospice care, what to expect at end of life, and how to best support their loved one. We also partner with facilities to offer education on end-of-life care, symptom management, and best practices—ensuring patients receive dignified, thoughtful care every step of the way.
